Transitions of Care for Patients with Chronic obstructive pulmonary disease (COPD) is organized by University of North Texas (UNT) Health Science Center Continuing Education and will be held during Feb 03, 2016 - Feb 03, 2018.

Conference Description :
Chronic obstructive pulmonary disease (COPD) is responsible for one death every four minutes in the United States and is now the third leading cause of death. COPD is also responsible for a significant number of potential preventable hospital readmissions, which result from poor transitions of care caused by breakdowns in communication, education or accountability. This presentation will address the elements of successful care transitions for patients with COPD.
Conference Objectives are :
• Discuss the impact of COPD on hospitalization.
• Describe the elements of successful care transitions.
• Evaluate the value of patient self-management in COPD.
