Transitioning to Adult Care for Youth with Chronic Health Conditions and Disabilities is organized by University of British Columbia Continuing Professional Development (UBC CPD).

Description::
This is a short online continuing medical education course designed to educate health providers on ways to better facilitate the continuity of care for youth with chronic health conditions and disabilities who are transitioning from pediatric care to the adult care system.

By the end of this course, you will be able to:

  • Recognize barriers to and risks at transition to adult care for youth with chronic health conditions and disabilities (CHC/Ds).
  • Identify practical strategies and/or tools to facilitate transition and transfer.
  • Identify common concerns of adolescents with disabilities or chronic illness that may be affected by their condition.
  • Identify challenges family physicians/primary care providers should anticipate in their young adult patients with CHC/Ds who are transitioning from paediatric to adult health care.
  • Identify effective approaches to building rapport with young adult patients with CHC/Ds.
  • Identify common barriers to adherence to medical care with patients with CHC/Ds.
  • Identify methods for promoting adherence and motivation in adolescent and young adult patients with CHC/Ds.
  • Identify key aspects of transition that are unique to the experiences of young adults with cognitive impairment/developmental disabilities and their families and caregivers.
  • Identify strategies family physicians/primary care providers can use to integrate young adults with significant developmental and intellectual disabilities into their practice.
  • Identify resources for addressing specific issues in transition for youth with significant developmental and intellectual disabilities such as decision-making, communication, adaptive functioning and health and health service utilization
