The purpose of this virtual kickoff event is to introduce the Target: Blood Pressure Program, explain how it supports the achievement of optimal blood pressure control, and help primary care providers and their care teams identify opportunities for improvement while implementing the M.A.P. Framework.
Learning Objectives
At the completion of this online course, the learner will be able to:
• Describe the impact of uncontrolled blood pressure (BP) from a public health perspective and a patient’s perspective.
• Identify evidence-based best practices your team can use to improve BP control: the M.A.P. checklists. •Describe key tools and resources your team can use to implement the M.A.P. checklists.
• Describe the real-world experiences of practices and health centers that have successfully implemented evidence-based checklists.
• Describe how achieving BP control in patients with hypertension is dependent on patient factors, physician factors, and system factors.
• Communicate about how team-based care can help improve systems that support great patient care. • List at least two sources of data your team can use to guide future improvement efforts.
• Demonstrate an understanding of the expectations of participating in the Target: BP program.
