The American Association of Critical-Care Nurses (AACN) is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s (ANCC's) Commission on Accreditation, ANCC Provider Number 0012 (60 min contact hour). AACN has been approved as a provider of continuing education in nursing by the California State Board of Nursing, California Provider number CEP01036 for 2.4 contact hours (50 min contact hour).
Activity Summary :
Severe sepsis kills more than 700 people a day in the United States. Many programs initiated around the septic patient fall short of desired outcomes. Sepsis was deemed a core measure by the Centers for Medicare and Medicaid Services in October 2015. The complexity of the disease, the variability in the presentation of the patient and the skill and knowledge level of the provider make this core measure distinctly different and potentially a challenge to implement. Providing the right care to the right patient at the right time across the continuum requires well-defined systems and processes. This workshop focuses on taking the participant through a four-tiered strategy for developing a program to identify patients early in the disease, whether prehospital, in a skilled nursing facility or within the hospital and to manage them based on current evidence.
Objectives :
• Summarize the four-tier process for effective sepsis program development and implementation across the continuum of care.
• Examine the evidence for the 3 hour and 6-hour core measure sepsis bundles.
• Evaluate potential barriers and effective resolution strategies for implementation and measurement of the sepsis bundles.
