Course Description: 
This session will look at how social disparities play a role with renal failure. It will go over how palliative care is part of the overall health care decision and how social disparities impact the decision-making process and options through identifying barriers in understanding and accessing hospice services.

Learning Objectives:

  • Explore palliative care in renal failure as a part of health care decision making.
  • Identify how social determinants of health impact this decision-making process.
  • Identify barriers exacerbating social disparities in accessing palliative care and/or hospice services.

Accreditation:
In support of improving patient care, the National Kidney Foundation is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American

Nurses Credentialing Center (ANCC), the American Academy of Physician Assistants (AAPA), the Commission on Dietetic Registration (CDR), and the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program to provide continuing education for the healthcare team.

Physician:
The National Kidney Foundation designates this enduring activity for a maximum of 1.5 AMA PRA Category 1 Credit(s)™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Nurse Practitioner, Nurse, Dialysis Technician:
Awarded 1.5 contact hours of continuing nursing education.

Physician Assistant:
National Kidney Foundation has been authorized by the American Academy of PAs (AAPA) to award AAPA Category 1 CME credit for activities planned in accordance with AAPA CME Criteria. This activity is designated for 1.5 AAPA Category 1 CME credits. Approval is valid until Sep. 6, 2025. PAs should only claim credit commensurate with the extent of their participation.

Dietitian:
As a Jointly Accredited Organization, the National Kidney Foundation is approved to offer dietitian continuing education by the Commission on Dietetic Registration (CDR). Organizations, not individual courses, are approved under this program. Dietitians completing this course receive 1.5 enduring continuing education credits.

Social Worker:
As a Jointly Accredited Organization, the National Kidney Foundation is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The National Kidney Foundation maintains responsibility for this course. Social workers completing this course receive 1.5 enduring continuing education credits.

Licensed social workers should contact their individual jurisdiction to review current continuing education requirements for licensure renewal.

The National Kidney Foundation is accredited as a provider of continuing transplant education by the American Board for Transplant Certification: CEPTC Provider Number #170. This activity has been granted 1.5 contact hours. Program ID 170-91.
