OVERVIEW

The content of this enduring activity is derived from expert-led neurology sessions presented at the American Neurology Summit 2025.

Sleep disorders are frequently encountered in neurology, and updated evidence-based guidelines from the American Academy of Sleep Medicine (AASM) have significantly reshaped the treatment landscape. Restless Legs Syndrome (RLS), REM Sleep Behavior Disorder (RBD), and central hypersomnia syndromes—including narcolepsy and idiopathic hypersomnia—require clinicians to understand recent therapeutic changes, safety considerations, and appropriate diagnostic workflows.

This CME activity equips neurologists with an updated, clinically practical understanding of these guidelines. Participants will review modern RLS management, including the shift away from dopamine agonists as first-line therapy due to high augmentation risk. New recommended strategies emphasize iron evaluation, prescription iron replacement, Alpha-2-delta ligands, and the role of advanced therapies such as IV iron, peroneal nerve stimulation, dipyridamole, and combination approaches.

The course also reviews updated treatment recommendations for RBD, focusing on melatonin titration, safety considerations with clonazepam, alternative agents in refractory cases, and critical counseling related to neurodegenerative risk. Additionally, the session covers the latest guidance for managing central hypersomnia, including first-line wake-promoting agents, the role of sodium oxybate and low-sodium oxybate formulations, pediatric approvals, and emerging therapies such as pitolisant and orexin agonists.

By the end of this activity, neurologists will be fully updated on practical, guideline-driven management strategies to enhance patient outcomes and safety across multiple sleep disorders.

Educational Objectives

After completing this activity, learners will be able to:

  • Interpret updated AASM guidelines for the treatment of Restless Legs Syndrome (RLS).
  • Apply AASM recommendations for the management of REM Sleep Behavior Disorder (RBD).
  • Explain guideline-based approaches to the treatment of central hypersomnia disorders, including narcolepsy and idiopathic hypersomnia.
  • Differentiate between first-line, second-line, and emerging treatment options within each disorder category.

Who Should Do This Course?

  • Neurologists treating patients with RLS, RBD, or hypersomnia disorders.
  • Sleep medicine specialists integrating updated AASM guidelines into clinical practice.
  • Providers managing refractory or complex sleep-related complaints.
  • Clinicians evaluating patients with narcolepsy, idiopathic hypersomnia, or parasomnias.
  • Healthcare professionals seeking evidence-based management strategies for sleep disorders.

Did You Know?

  • RLS affects ~10% of adults, with augmentation occurring in over 40% of long-term dopamine agonist users. (Sleep Medicine Reviews, 2021)
  • RBD is one of the strongest prodromal markers of synucleinopathies, with over 70% converting to Parkinsonian disorders within 15 years. (Lancet Neurology, 2020)
  • IV iron therapy has demonstrated significant improvement in moderate-to-severe RLS, especially when oral iron fails.(Neurology, 2019)
  • Narcolepsy Type 1 involves >90% loss of orexin neurons, guiding the development of orexin-2 receptor agonists.(Nature Reviews Neurology, 2022)
  • Idiopathic hypersomnia patients may sleep >11 hours per day, and many do not meet MSLT criteria—contributing to widespread underdiagnosis.(Sleep, 2020)
  • Solriamfetol improves daytime wakefulness by up to 12 MWT points, outperforming placebo.(JCSM, 2019)
The estimated time to complete this activity is 1 hour.
