Safer Transitions, Fewer Re-Hospitalizations with PAH: An Interdisciplinary Guide is organized by Prime Education, LLC and will be held from Jun 15, 2016 - Jun 14, 2019.

Overview:
Patients with pulmonary arterial hypertension (PAH) often undergo multiple transitions throughout the course of their disease, beginning with the journey to achieve an accurate diagnosis and initiate appropriate therapy. The complexity of the disease and therapy require ongoing interprofessional coordination and system-based approaches to effectively manage care transitions to minimize hospital readmissions and optimize patient outcomes. In partnership with the National Transitions of Care Coalition (NTOCC) and Case Management Society of America (CMSA), a transitions of care white paper has been developed for system leaders and interprofessional clinical team members who treat and manage patients with PAH. The guidance paper, developed by a multidisciplinary advisory board incorporates essential interventions and pathways designed to ensure effective and safe transitions of care for patients with PAH across healthcare settings.

• Implement safe and effective transitions of care strategies to improve treatment outcomes for patients with PAH
