Goal:
To inform and educate pharmacists about the management of patients who require reversal of direct oral anticoagulants (DOACs) because of major bleeding, emergency surgery, or an urgent procedure; educated pharmacists on management of these patients, including interpreting laboratory assays and use of pharmacologic agents; and review investigational agents for DOAC reversal.

Learning Objectives:
Upon completion of this program, participants should be better able to:
• Identify rates of major bleeding and risk factors for bleeding in patients taking direct oral anticoagulants (DOACs).
• Identify the qualitative and quantitative blood coagulation tests for DOACs.
• Explain the mechanisms of action of idarucizumab and andexanet alfa for reversing bleeding associated with DOAC therapy.
• Discuss the appropriate dosing and administration of idarucizumab for dabigatran-associated bleeding reversal.
• Discuss the appropriate dosing and administration of andexanet alfa for factor Xa inhibitor reversal.
• Review the appropriate use of alternative agents used for reversal of factor Xa inhibitors.
• List investigational agents for DOAC reversal.
