Patient Centered Transition: Smoothing the Journey from Pediatric to Adult Care is organized by ABcomm, Inc. and will be held from Mar 30, 2018 - Mar 30, 2020.

The target audience for this medical event is multidisciplinary CF Treatment Team members; physicians (pediatric and adult pulmonologists and pediatricians), nurse practitioners, physician assistants, nurses, respiratory therapists, social workers, dieticians, counselor, and center coordinators. This CME Conference has been approved for a maximum of 1 AMA PRA Category 1 Credits.

Course Description:
This activity explores components of successful transfer of teens to adult care, prepping younger patients earlier for transfer, empowering CF patients to take control of their own treatment, utilization of adherence and transfer tools including CF RISE, discussion of adherence issues based on developmental stages (pre-teen, teen, young adult, adult), and motivational interviewing directed at patient goal obtainment.?

Course Objectives:
• Describe the benefits and challenges associated with the transition from pediatric to adult care among individuals with CF.
• Examine current models of transitional care from the pediatric to adult CF setting.
• Discuss strategies and tools for empowering people with CF to manage their own care.
