Description:
Cardiovascular–kidney–metabolic (CKM) syndrome drives substantial residual risk for cardiovascular events and kidney disease progression despite guideline-directed medical therapy (GDMT). This session provides a practical, case-based overview of applying current guidelines and emerging evidence to optimize CKM care using a multimodal, team-based approach. Faculty will highlight key guideline recommendations and recent clinical trial data supporting early, comprehensive GDMT, with a focus on integrating aldosterone-targeted therapies, including nonsteroidal mineralocorticoid receptor antagonists and investigational aldosterone synthase inhibitors, to address persistent cardiokidney risk. Attendees will also learn strategies for care coordination, improving adherence, and overcoming system-level barriers. Through clinical cases and practical frameworks, participants will gain actionable approaches to implementing evolving therapies in real-world CKM care.

In support of improving patient care, the National Kidney Foundation is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
Physician:
The National Kidney Foundation designates this enduring activity for a maximum of 1.5 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.5 MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
Pharmacist:
This knowledge-based enduring activity has been assigned Universal Activity Number, JA0004461-0000-26-012-H01-P, for 1.5 contact hours (0.15 CEUs). To receive credit, learners must read the activity overview, learning objectives and faculty disclosures, complete the pre-test, review the content, complete the post-test with a minimum score of 60% (post-test may be taken up to 3 times) and electronic activity evaluation.
Physician Assistant:
National Kidney Foundation has been authorized by the American Academy of PAs (AAPA) to award AAPA Category 1 CME credit for activities planned in accordance with AAPA CME Criteria. This activity is designated for 1.5 AAPA Category 1 CME credits. Approval is valid until July 7, 2027. PAs should only claim credit commensurate with the extent of their participation.

Awarded 1.5 contact hours of continuing nursing education (includes 0.75 contact hours in the area of pharmacology).
Dietitian:
As a Jointly Accredited Organization, the National Kidney Foundation is approved to offer dietitian continuing education by the Commission on Dietetic Registration (CDR). Organizations, not individual courses, are approved under this program. Dietitians completing this course receive 1.5 continuing education credits.
