Course Description:
The pharmacist plays an integral part in minimizing and preventing medication-related problems. Medication Therapy Management (MTM) is patient care provided by a pharmacist aimed to optimize drug therapy and improve therapeutic outcomes for patients. Pharmacists should have an understanding of the core elements of MTM, MTM provisions of Medicare Part D benefits and considerations when developing MTM services. Use of appropriate communication skills and accurate assessment techniques is key during an MTM visit. Using a case study patient with hypertension, this webinar will review the MTM process including patient assessment, identifying medication-related problems and making recommendations based on treatment guidelines.
