The field of traumatic stress is at an interesting point in its history with notable advances in the scientific understanding of significant factors that impact psychological adjustment following trauma exposure. However, there is a proceeded struggle to elucidate this large and expanding evidence-base to prepare to meet most effectively the requirements of those exposed to trauma. This gap is particularly apparent for the hardest to reach and most vulnerable populations. That is, despite a proliferation of research, we know that most trauma-exposed individuals will not reach evidence-based support. Some of the challenges to broader distribution and utilisation of existing scientific knowledge incorporate high burden connected with training clinicians; feasibility challenges for patients seeking to join in evidence-based care; treatment strategies that are complex, burdensome and lengthy; and cross-cultural differences that conflict with treatment acceptability and feasibility in certain populations.
