Description:
In this online learning module, participants will learn about why it is important to have and document Goals of Care discussions in a systematic way in the primary care setting for patients with complex care management needs.
They will learn about a team-based ThedaCare pilot initiative designed to identify patients at high-risk of misalignments in plans of care and facilitate Goals of Care discussions and appropriate continuing services and/or onward referrals.
Participants will learn how such discussions can be documented and reviewed using a novel Goals of Care flowsheet in the electronic health record, as well as how to have appropriate, comprehensive, compassionate, and patient and family centered Goals of Care discussions.
Learning Objectives:
As a result of participation in this educational series, members of the healthcare team will be able to:
• List at least two reasons why it is important to have and document Goals of Care discussions with patients and, where appropriate, families and/or surrogate decision makers.
• Describe two circumstances in which having a Goals of Care discussion might be helpful and appropriate.
• Describe the tool used to document Goals of Care in the electronic health record in a consistent way that is visible to all health providers across settings.
• Demonstrate improved comfort having and documenting Goals of Care discussions.

American Board of Internal Medicine (ABIM)
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 0.75 Medical Knowledge MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
