Description:
Gestational diabetes mellitus (GDM) affects approximately 300,000 women annually. The proposed International Association of Diabetes and Pregnancy Study Groups (IADPSG) diagnostic criteria would increase this number significantly. Mark B. Landon, M.D., discusses appropriate fetal surveillance in GDM patients and how to integrate the use of dietary therapy, exercise, the indications for insulin and oral hypoglycemic agents and strategies for monitoring maternal glucose control.

Objectives:
• Describe the metabolic changes in pregnancy that produce a “diabetogenic stress” and insulin resistance.
• Identify the short- and long-term morbidities for the woman with GDM and her infant.
• Construct an algorithm to detect GDM using methods currently recommended for screening and diagnosis.
• Integrate the use of dietary therapy, exercise, the indications for insulin and oral hypoglycemic agents and strategies for monitoring maternal glucose control.
