You will need to review all of this course content, then receive a 70% or higher passing grade on a quiz, and lastly complete an evaluation to receive your certificate and complete this course. As stated in the ARN Core Curriculum, care transitions are “The movement of patients with acute or chronic illnesses across the continuum of care between various healthcare settings as care needs change.” This course provides valuable teachings so you have the knowledge to facilitate effective care transitions for your rehabilitation patients. In addition, the program will guide you on the development of interprofessional relationships and fostering effective interprofessional collaboration.
