A crucial role in all cardiovascular prevention strategies is played by the treatment of dyslipidemia, especially hypercholesterolemia.

The evidence available today demonstrating how the reduction in plasma levels of LDL cholesterol (LDL-C) reduces cardiovascular risk is extensive and unequivocal: a reduction of 40 mg/dl in LDL-C is associated with a reduction of 20-25 % mortality from cardiovascular causes and nonfatal myocardial infarction.

This unique and consistent evidence now recognizes LDL-C as the lipid component with the greatest documented pathogenetic effect on coronary events and therefore the most important target of both therapy and prevention strategies.

Today, clinicians have a new therapeutic weapon in their hands, the first-in-class drug inclisiran (siRNA > small-interfering RNA). This highly innovative therapy represents a real revolution in the treatment and care of patients with high and very high-risk atherosclerotic cardiovascular disease. Inclisiran through the ORION Program trials not only demonstrated good safety and tolerability, but above all it demonstrated an effective reduction (up to 52%) of low-density lipoprotein cholesterol (LDL-C) in patients with high LDL-C levels. Another important innovation is represented by the administration on a six-monthly basis which will guarantee optimal adherence and persistence to the therapy.
