Description:
This session describes the use of extracorporeal membrane oxygenation (ECMO) for the management of adults with severe acute respiratory failure. Included is a review of traditional indications for ECMO for respiratory failure (ARDS, H1N1 influenza, pneumonia), as well as expanded criteria for the application of this rescue strategy for patients failing traditional management, including acute traumatic injury, support for post-lung transplant patients with primary graft dysfunction and bridge-to-lung transplant therapy. The technical aspects of ECMO are reviewed as they apply to nursing management, including cannulation, modes of therapy and circuit design. The current evidence to support the use of ECMO for respiratory failure is reviewed. Cases are presented to highlight risks of therapy, and guide recognition and management of hemodynamic and gas exchange problems, bleeding complications and readiness for weaning from support.

• Discuss common indications and criteria for the use of ECMO in the treatment of severe acute hypoxemic respiratory failure and ARDS.
• Describe ECMO cannulation techniques, risks of therapy, management of complications, and population-specific outcomes.
• Analyze specific ECMO patient case scenarios using algorithms for managing hypoxemia, bleeding, recirculation and weaning strategies.
