Anemia is a condition characterized by a reduced production of red blood cells, which compromises the supply of oxygen to the body. In patients with chronic kidney disease, more commonly known as chronic renal failure, anemia is one of the most frequent complications, which manifests itself already in the early stages of the disease, contributing to the progression of cardio-renal damage and to the reduced quality of life of the nephropathic patient.
Anemia can affect both patients undergoing conservative therapy and those undergoing hemodialysis or peritoneal dialysis, and also kidney transplant recipients. In Europe, where approximately 10% of the population suffers from chronic kidney disease and of these one in three develops the anaemic complication. The high frequency of anemia is caused by the progressive reduction of the functioning renal parenchyma in renal failure for which the kidneys do not produce a sufficient quantity of erythropoietin, the hormone that stimulates the production of red blood cells. As a consequence of erythropoietin deficiency, the number of red blood cells is reduced with a reduced capacity of the blood to transport oxygen to the body's tissues. In fact, chronic kidney disease anemia is not only secondary to reduced renal production of erythropoietin; inflammation and reduced availability of iron are in fact additional and important causal factors of anemia in renal failure.
HIF PH inhibitor drugs are a pharmacological replacement therapy for a multifactorial para-physiological therapy and can be applied to patients in the conservative phase and on dialysis who, due to the inflammatory state typical of chronic kidney disease that reaches its maximum expression in dialysis, do not respond to erythropoietin and remain anaemic despite even high, and potentially harmful, doses of erythropoietin.
