Description:
The goals of population management are to reduce measurably the complications of selected chronic diseases and to improve the quality of life for patients in such management programs. Many programs are telephonic and involve registered nurses. Some include other healthcare providers, such as LPNs/LVNs, pharmacists, registered dietitians, and social workers. Nurse case managers may provide in-home visits, arrange for durable medical equipment, and coordinate other care. Population health management also encompasses the patients’ overall well-being and not just physical functions. This module discusses disease and population management and the role nurses play.

Goal and Learning Outcomes:
The goal of this continuing education program is to provide nurses with information about population management and how standardized practice guidelines can improve the quality of life for patients with chronic diseases. After studying the information presented here, you will be able to:
• List the three characteristics of population management programs
• Identify five chronic diseases commonly in population management programs
• Discuss educational nursing interventions for asthma, diabetes, and heart failure
