Whether you’re working toward PCMH transformation or sustaining your PCMH through annual reporting, knowing how to accurately collect and report patient data is critical to quality measurement and continuous improvement. During this webinar, faculty will discuss how to select populations, reporting periods, timeframes, numerators, denominators and more. This webinar was recorded on December 3, 2019.
