Objectives
•Achieve benchmarks for national heart failure quality measures, including appropriate diagnosis, evidenced-based treatment of acute and chronic heart failure, reduced hospital readmissions, and patient-centered clinical outcomes.
•Implement guideline-directed methods for heart failure risk stratification based on patients’ hemodynamic characteristics, including ejection fraction and blood pressure, and comorbidities including renal disease.
•Determine precipitating causes of acute heart failure episodes to guide timely and appropriate treatment decisions.
•Summarize key evidence for the accuracy and utility of established and emerging biomarkers for the timely diagnosis of heart failure.
•Apply current evidence-based guidelines and expert consensus approaches to making individualized treatment decisions regarding conventional and new therapy for acute and chronic heart failure.
•Apply consensus-based and evidenced-based methods for ensuring effective care coordination to improve quality and clinical outcomes in heart failure.
