Objectives
•Describe recent updates to ADA standards of care and guideline recommendations for the evidence-based management of diabetes
•Translate to practice the latest advances within diabetes research, including clinical trial data on novel insulin-based therapies and timely intensification with insulin-based therapies
•Summarize the potential impact of the PCMH model and Diabetes Recognition Program (DRP) on diabetes care
•Implement practice-specific strategies that are patient-centered and team-based in an effort to improve the quality of diabetes care resulting in more responsive to individual patient preferences and needs.
