Description:
This two-day virtual workshop is designed to help you successfully address current medication safety challenges that impact patient safety. Program faculty will provide you with the specific tools and resources needed to establish and sustain an aggressive, yet focused medication safety program.

Decades of data and learning from ISMP's Medication Error Reporting Programs make this a unique experience you will not find anywhere else. You will participate in hands-on practice in error analysis, evaluate root causes related to errors, learn how to effectively select high-leverage strategies, and use data to help sustain safety efforts within your organization.

Learning Objectives:
Following completion of this workshop, participants will be able to:

• Perform system-based medication error analysis.
• Identify latent and active failures that lead to medication error events.
• Develop error-reduction strategies for the use of high-alert medications.
• Utilize various data collection methods and tools to identify medication-related risks.
• Discuss the role and responsibilities of the medication safety officer or individual(s) responsible for medication safety oversight.
• Identify human error, at-risk behavior, and reckless behaviors.
• Describe appropriate organizational responses to behavioral choices.
• Measure the effectiveness of selected error-reduction strategies.
