This practical, encouraging session shows Aging Life Care Professionals how to track meaningful outcomes using simple tools, recognize patterns over time, and tell a clear story about impact. Designed for practitioners who feel unsure where to start, this presentation and companion workbook offer an easy 1-2-3 framework that strengthens care, builds confidence, and makes the value of Aging Life Care visible.

Measuring outcomes can feel intimidating or disconnected from day-to-day practice, especially in busy Aging Life Care settings where urgent needs take priority. This session demystifies outcome measurement and shows how it can be simple and closely aligned with the real work of supporting clients and families.

Participants will learn a straightforward 1-2-3 approach to outcome measurement: selecting one meaningful outcome, using a simple tool to gather information, and looking for patterns over time to tell a clear story of impact. The presentation is paired with a companion workbook designed to help participants stay on track, reflect, and apply concepts at their own pace after the session.

Drawing on practice examples and findings from the Virginia Tech study, this session highlights outcomes that matter most to clients and families and demonstrates how outcome data can strengthen care planning, professional credibility, and referral relationships.

Goal:
The goal of this webinar is to empower Aging Life Care Managers with up-to-date, clinically informed insight into the most common cardiac conditions that emerge with age—while spotlighting newly identified risks that disproportionately affect younger women. By strengthening their understanding of these developments, Aging Life Care Managers can better guide clients in recognizing warning signs, asking the right questions, and making informed decisions about their heart health.

Describe the purpose and value of outcome measurement in Aging Life Care practice for clients, families, referral partners, and professional credibility.
Apply a simple 1-2-3 framework to outcome measurement by selecting one outcome, using a brief tool to gather information, and identifying patterns over time.
Differentiate between formally validated research tools and practice-informed tools, and identify when each type is appropriate for use in Aging Life Care practice.
Explain how qualitative narratives and quantitative data can be combined to create a fuller picture of client and family impact.
Recognize ethical considerations related to collecting, using, and sharing outcome data, including confidentiality, consent, and use of aggregate information.
