Part 1: HER2-Low and HER2-Ultralow Breast Cancer

Learning Objectives:
Upon completion of this activity, participants should be able to

  • Appreciate the incidence and pathological and molecular characteristics of HER2-low and HER2-ultralow breast cancer, and understand the implications of diagnosis of these phenotypes for routine patient management.
  • Assess the biological rationale for the use of HER2-directed therapy for patients with HER2-low and HER2-ultralow breast cancer, and determine its current clinical utility in this disease subset.
  • Appraise available clinical research establishing the safety and efficacy of HER2-directed antibody-drug conjugate (ADC) therapy for patients with hormone receptor (HR)-positive and HR-negative HER2-low metastatic breast cancer, and optimally incorporate this strategy into treatment algorithms.
  • Recognize Phase III efficacy results establishing the benefit of HER2-directed ADC therapy for patients with HR-positive, HER2-ultralow metastatic breast cancer, and consider the potential role of this novel approach.
  • Understand the spectrum, frequency and severity of treatment-emergent adverse events associated with HER2-directed ADCs, and develop strategies to monitor for, prevent and manage these complications.
  • Recall the design of ongoing clinical trials evaluating novel strategies for HER2-low and HER2-ultralow breast cancer, and counsel appropriate patients about availability and participation.

Part 2: Endocrine-Based Therapy

Learning Objectives:
Upon completion of this activity, participants should be able to

  • Consider available clinical trial findings with CDK4/6 inhibitors for localized hormone receptor (HR)-positive, HER2-negative breast cancer, and identify patients for whom adjuvant treatment with one of these agents may be appropriate.
  • Evaluate biological, patient and treatment-related factors to personalize the selection and sequencing of therapy for HR-positive, HER2-negative metastatic BC (mBC).
  • Review available research documenting the correlation between the presence of various biomarkers (eg, PIK3CA/AKT1/PTEN alterations, ESR1 mutations) and response to specific therapies, and develop optimal molecular testing algorithms for patients with HR-positive mBC.
  • Appraise published efficacy and safety data from randomized clinical trials evaluating CDK4/6 inhibitors for HR-positive mBC, and use this information to appropriately counsel patients regarding the optimal clinical use of these agents.
  • Recall the frequency of phosphoinositide-3 kinase pathway mutations in patients with HR-positive mBC, and employ evidence-based approaches to targeting these abnormalities in patients with PIK3CA-mutated disease.
  • Understand the mechanism of action of, published research findings with and current and future clinical roles for oral SERDs (selective estrogen receptor degraders) for patients with HR-positive mBC.
  • Interrogate published Phase III research documenting the efficacy of AKT inhibitors for patients with progressive HR-positive mBC to determine the current clinical applicability of this approach.
  • Recognize the side effects and toxicities associated with available and investigational endocrine- based therapies used in the care of patients with breast cancer, and identify management and mitigation strategies.

Part 3: Metastatic Breast Cancer

Upon completion of this activity, participants should be able to

  • Consider biological, patient, psychosocial and treatment-related factors to personalize the selection and sequencing of therapy for metastatic breast cancer (mBC).
  • Review and use published research findings to effectively inform the selection and sequencing of available therapeutic agents and regimens for patients with HER2-positive mBC.
  • Review published research data supporting the use of chemotherapy in combination with anti-PD-1/PD-L1 antibodies for patients with metastatic triple-negative breast cancer (mTNBC), and use this information to make appropriate treatment recommendations.
  • Evaluate published research findings, clinical factors (eg, age, performance status, prior therapeutic exposure, HER2-low status) and personal preferences in the selection and sequencing of available therapeutic agents for patients with PD-L1-negative mTNBC or those with PD-L1-positive mTNBC who experience disease progression on front-line chemoimmunotherapy.
  • Discuss available research establishing the efficacy of PARP inhibitors in patients with mBC harboring BRCA or other homologous recombination repair pathway mutations, and identify appropriate candidates for treatment with these agents.
  • Define endocrine resistance in patients with HR-positive mBC, and integrate available nonhormonal therapies into the management of relapsed/refractory disease.
  • Review published Phase III research documenting the efficacy of TROP2-directed antibody-drug conjugates for mBC to determine the current and potential clinical applicability of this approach.
  • Assess the biological rationale for, early data with and ongoing clinical trials evaluating other novel agents and treatment strategies in development for mBC.
