In this discussion, panelists will explore the dynamic world of multi-payer alignment. Guided by Executive Forum Co-Chair Dr. Alice Chen, panelists will address the leading role of national and regional stakeholders in driving directional alignment across value-based care programs.Real-world success stories illuminate the strategies and factors, such as the pivotal role played by data infrastructure, that have made some alignment initiatives effective. The success of these initiatives not only lay the foundation for success in payment reform but also promote burden reduction and elevate the quality of patient care. Additionally, this panel will unveil the National Health Plan Workgroup, a significant step toward achieving the multi-payer alignment needed to advance health care reform.

Enabling Action at the Community Level: Standards for Engaging CBOs in Alternative Payment and Delivery Models

Community-based organizations (CBOs) are vital in advancing health equity within alternative payment models (APMs). Patients with health-related social needs often have strong, established connections with local CBOs, and APMs can foster meaningful relationships between health systems and CBOs.Moderated by Professor of Healthcare Ethics at the University of Chicago and Health Equity Advisory Team (HEAT) Co-Chair Dr. Marshall Chin, and Market President of AmeriHealth Caritas District of Columbia and HEAT Co-Chair Karen Dale, the panel will provide insights about how payers, health systems, and CBOs have successfully navigated societal barriers to establish impactful partnerships that benefit patient populations. Panelists will reveal the essential standards that underpin successful partnerships that can empower communities and bridge the gap between health care and social services.

Lunch Session One: Considerations and Challenges When Implementing Value-Based and Accountable Care Models

Join CMMI Deputy Director Arrah Tabe-Bedward and a panel of health plans as they discuss the operational considerations and challenges encountered when implementing value-based and accountable care models. Learn their tips and tactics for handling said challenges.

Lunch Session Two: Q&A with CMMI Leadership: Providing Accountable Care for Complex Populations

Join CMMI’s Deputy Director, Ellen Lukens and Chief Strategy Officer, Purva Rawal as they speak to key CMMI priorities in providing accountable care for complex populations. Public and private payers are actively rolling out APMs that increasingly focus on populations with complex needs. The industry recognizes an increasing need to integrate specialty care providers into new and existing value-based and accountable care models. CMMI recently announced three new models related to the role of integrated behavioral health in advanced primary care, community-based dementia care, and advanced health equity at the state level. Additionally, the Innovation Center is making specialty care integration a key feature of new models and its broader strategy. Join us as we discuss what it will take for these models to improve outcomes for target populations and engage new providers.

Enabling Action Beyond Payment Reform: Advancing System Transformation Through Accountable Care

Within prospective population-based payment models, payers and providers leverage inherent flexibilities to innovate and improve patient care. This panel is moderated by Health Care Transformation Task Force Executive Director and Accountable Care Action Collaborative (ACAC) co-chair Jeff Micklos. Hear from payers and providers using rapid cycle learning and evaluation techniques to pinpoint effective strategies for improving health outcomes and enhancing patient experience. Panelists will also discuss how these initiatives uncover care gaps and identify crucial areas where additional efforts are essential to deliver accountable care that benefits patients.

Enabling Action with the Individual at Top of Mind: Effective Integration of Patient and Caregiver Perspectives

This session emphasizes the importance of understanding the perspectives of the individuals who receive care and their caregivers in achieving the intended person-centered care goals of health care delivery and payment reform. In pursuing person-centered care, providers, payers, purchasers, and health care organizations must genuinely engage with those receiving care and their families to grasp the key drivers of health in communities.Panelists and moderator Libby Hoy, Founder and CEO of PFCCpartners, will showcase real-world examples of engagement across payers, providers, and health care systems, offering constructive viewpoints into the profound impact of putting individuals at the forefront of health care decision-making. The panel will also introduce the LAN’s Person Perspectives Council, which provide critical guidance on the challenges affecting individuals receiving care and caregivers in value-based care relationships.

Reflecting on Action: 2023 APM Measurement Effort: Advancement in APM Adoptions and the Impact on Covered Lives

Catalyst for Payment Reform Program Director Andrea Caballero and panelists discuss the outcomes of the 2023 LAN alternative payment model (APM) Measurement Survey. The survey’s results demonstrate the industry-wide progress in APM adoption. Speakers will also discuss the newest, groundbreaking metric on covered lives, and the urgency surrounding covered lives measurement and the imperative to address identified gaps.Engage with panelists as they reflect on the data, emphasize the critical connection between these findings and the overarching goal of tackling health disparities, and generate momentum for response.
