As high implant survival and success rates have been reported, the esthetic outcome has become the main focus of interest in esthetically sensitive areas. After 30 years in implant dentistry our treatment protocols have evolved and are ever-changing based on scientific evidence and literature with clinical experience. Innovation and change has kept us striving for ideal treatment outcomes. This presentation will illustrate classic concepts and techniques in a modern world. The team approach utilizing interdisciplinary treatment with long-term outcomes will be demonstrated. Dentistry in the third dimension with CBCT and intraoral scanning to virtual treatment planning and guided surgery for “every patient every time” will be discussed. A review of static vs. dynamic guided surgery, milled vs. 3D printed technology with case presentations and the latest literature review to help the clinician choose patient/site specific treatment protocols. A discussion of site development with the use of biologics for regeneration for simple to complex cases will be illustrated. The prosthetic reconstruction should imitate the appearance of the natural teeth. Clinical cases will illustrate a stepwise approach of tissue manipulation with the use of provisional restorations to create a subgingival contoured sulcus. With the latest technology, through guided prosthetics a virtual abutment can achieve natural root replacement contours. The purpose of this presentation is to discuss advancements in machined abutment design: pre-contoured versus CAD/CAM, metal versus ceramic abutments. Decision criteria will be formulated in order to choose between all ceramic abutments and crowns as well as titanium, gold abutments and ceramometal crowns. The clinician will have a better understanding of abutment selection to achieve the ultimate esthetic integration.
