OVERVIEW

Advance Directives and Provider Orders for Life-Sustaining Treatment Facilitator Training is organized by Hawaii Pacific Health (HPH) and will be held on Aug 26, 2026 at Wilcox Medical Center, Lihue, Hawaii, United States of America.

Course Objectives:

  1. Apply advance care planning resources to promote advance care planning discussions.
  2. Engage the multi-disciplinary team to patient-centered discussion addressing advance care planning.
  3. Promote patients’ knowledge of advance directives, including healthcare Power of Attorney designation.
  4. Implement correct documentation on of Provider Orders for Life-Sustaining Treatment (POLST)-Cardiopulmonary Resuscitation on (CPR), Do Not Attempt Resuscitation on (DNAR), full and limited treatment, me-limited trials and Comfort Care.
  5. Promote correct process of ACP documents, access and utilization on in your organization.
  6. Identify end-of-life options including Hawai‘i’ requirements on Medical Aid in Dying, Our Care, Our Choice Act.
