Recent years have seen a radical shift in the epidemiology of chronic liver disease that can lead to liver cirrhosis. Once intractable, the focus is now on eradicating the hepatitis C virus, while the new therapy for hepatitis delta virus makes this group of patients curable. One in three people suffers from metabolically related hepatic steatosis, which can progress to liver cirrhosis and hepatocellular carcinoma. It is clear that chronic liver disease screening should be performed within community health care settings, including the management of patients with decompensated liver cirrhosis when this condition does not require hospitalization. Primary liver tumors represent the iceberg of chronic liver disease. Hepatocellular carcinoma accounts for 90% of cases and affects approximately 13,000 Italians each year. Unfortunately, this tumor is characterized by poor survival rates at both 5 years (approximately 20% of patients) and 10 years (10% of patients), making it one of the top five causes of cancer-related death. Compared to even a few years ago, referral centers for this disease have a wider range of treatment options (liver resection and transplantation, local-regional therapies, systemic therapies) available to treat this disease, but these necessarily require early diagnosis to lead to oncological cure.

The purpose of the conference is precisely to emphasize the need for close liaison between community health and hospitals to identify patients with chronic liver disease in light of new epidemiological evidence and to jointly manage the complications of cirrhosis.
