Prescriptive appropriateness is a fundamental public health objective in every therapeutic context.
This is even more true in the field of chronic pain therapy, in light of Article 1 of Law 38/2010 “Provisions to facilitate access to pain therapy and palliative care”, especially for those patients who, in addition to the fragility caused by painful disease, also face problems connected to the presence of heart disease. Chronic pain represents not only a serious issue of healthcare ethics and social equity but also a heavy burden for national economies. In times such as these, characterized by a negative economic spiral and strong pressure to cut social and healthcare costs, it is important to pay attention to the economic dimension of pain.

The pain–heart disease combination is frequently present in particular patient categories: elderly individuals with musculoskeletal pain typical of aging, postmenopausal women, and patients who, following an oncological diagnosis, undergo or have undergone cardiotoxic chemotherapy.

A correct pharmacological approach to chronic pain in these “fragile” patients should involve an accurate diagnosis of the type of pain and early treatment to prevent chronicity. In managing moderate and/or severe pain, the percentage of patients treated with opioids, as recommended by guidelines, is very low compared to that of NSAID use.

The course aims to address the issues related to the need for the appropriate, combined, and simultaneous use of multimodal pharmacological therapies for the treatment of pain and cardiovascular diseases in patients suffering from such comorbidities, both in general practice and in specialist centers.

Special attention will be given to the doctor–patient relationship, within the framework of the universally accepted concept that communication itself is a tool of care.
