Accreditation Statement
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Council on Optometric Practitioner Education (COPE). Visionary Medical Education (VME) is accredited by COPE to provide optometrists with independent continuing medical education required by optometric regulatory boards to ensure the continuing competence of licensure.
Credit Designation Statement
VME designates this synchronous in-person activity for a maximum of 12 COPE CE credits. Optometrists should claim only the credit commensurate with the extent of their participation in the activity.

Attendee Registration Terms & Conditions
By submitting this registration form to attend the INSIGHT 2024 Conference by Eyes on Eyecare (the “Event”), you, the individual submitting this Registration (“Attendee”), hereby agree to the terms and conditions set forth below (the “Terms and Conditions”). These Terms and Conditions apply to each Attendee registered for the Event. In addition to these Terms and Conditions, Eyes On Eyecare reserves the right to establish further regulations as may be deemed necessary to the general success and well-being of the Event and/or any person(s), including Attendees, affiliated therewith. By signing below, you acknowledge and agree to the Terms and Conditions herein.
- Registration:
- Standard Registration (January 17, 2024 - April 17, 2024): $299 per person. Please note that registering with a Purchase Order will hold your place, but PAYMENT MUST BE MADE IN FULL within 10 days of the Standard Registration purchase date to secure the Standard Registration rate. If you register within 20 days of the conference, all payments must be received no later than April 17, 2024, at 11:00am PDT or On-Site Registration fees will apply.
- On-Site Registration (April 18, 2024 - April 20, 2024): $399 per person. On-site registration will be available for walk-in participants April 19, 2024 - April 20, 2024 in the lobby of the Oquendo Center in Las Vegas. Payment must be made in full at the time of registration.
- Payment. By submitting this registration, Attendee(s) agrees to pay, in full, for each registration phase pursuant to the terms set forth in the confirmation email (also listed below). Failure to make timely payment as provided herein does not release you from your obligation to pay the registration fees in full, and Eyes on Eyecare does not waive its rights to collect as a result of any failure to strictly enforce payment deadlines.
If payment is not received in full within 10 days of registration or by April 1, 2024, Attendee(s) may, in Eyes On Eyecare's sole discretion, forfeit its right to the ‘Standard Registration' rate and/or its right to attend the Event as well as any amounts previously paid for registration to the Event. Registering with a Purchase Order will hold your place, but PAYMENT MUST BE MADE within 10 days of the Standard Registration purchase date or by April 1, 2024. If you register within 20 days of the conference all payments must be received no later than April 17, 2024 at 11:00am PST, or On-Site Registration fees will apply. If payment is not received in full by the close of the deadline, the On-Site Registration rate will apply. Check payment(s) must be submitted using the registration confirmation you receive at checkout. Eyes On Eyecare is not required to issue a separate invoice or otherwise provide an additional receipt to the Attendee.
Any payments made by Check must submit the registration confirmation provided to the account holder and/or Attendee(s) upon checkout. An email confirmation of registration shall be sent to the email address provided by the account holder and/or Attendee(s). The email confirmation serves as the Attendee's invoice for the Event. Eyes On Eyecare is and shall not be required to issue a separate invoice or otherwise provide an additional receipt to the Attendee. If this is required, please email Michele@CovalentCreative.com.

- Attendee Cancellation & Refund Policy.
- Cancellation & Substitution. Event registrations may be canceled and fully refunded if written requests are received on or before April 15, 2024. Please note that Eyes On Eyecare is unable to make exceptions for emergencies, extreme weather, pandemics, natural disasters, or labor strikes. However, Attendee may send someone in Attendee's place, subject to these Terms and Conditions. For registration questions, please email events@CovalentCreative.com. Please note that registration(s) cannot be transferred to a future Eyes On Eyecare event.
- Refund Policy. Refunds will be issued in full for refund requests received on or before April 15, 2024. After April 15, 2024, no refunds will be issued. If the event is rescheduled, all purchases will be honored for the rescheduled event date or may be refunded in full. If the event is canceled, all purchases will be refunded in full. After submitting your refund request in writing by email to Michele@CovalentCreative.com, please allow up to five (5) business days to process your refund request.
- Event Cancellation, Postponement, Rescheduling. Eyes On Eyecare reserves the right to cancel, postpone, or reschedule the Event at any time, at its sole discretion. In the event of cancellation, postponement, or rescheduling, Eyes On Eyecare will make commercially reasonable efforts to post details of any cancellation on the Event website. Attendees are responsible for checking this information prior to the Event.
- Attendance Rules. By attending the Event, Attendee agrees to abide by all generally applicable rules and regulations governing the Event that have been or hereafter will be provided or otherwise made known to you by Eyes On Eyecare. Attendees are further required to comply with the rules and regulations governing the applicable venue. The Attendee shall insure and keep insured any property they bring onto the venue, and any such property is brought onto, or left, at the venue at their own risk.
- Liability. Subject to the other provisions of this section, Eyes On Eyecare shall only be liable to Attendees for proven direct losses suffered due to the gross negligent acts or omissions of Eyes On Eyecare. Eyes On Eyecare's liability for such proven direct losses is limited in total to the amount paid by the Attendee to Eyes On Eyecare to attend the Event. Eyes On Eyecare expressly excludes any liability for any property damage or loss incurred at any time during the Event. Eyes On Eyecare shall not be liable (whether such liability arises due to negligence, breach of contract, misrepresentation, or otherwise) for any direct losses or damages not stated above or any indirect or consequential losses or damages however arising. This liability exclusion shall include, but not be limited to (and whether direct, indirect or consequential), loss of use or any profit, business, or data or any loss of use or damage suffered by any Attendee, person or entity, as a result of an action brought by a third party, even if such loss was reasonably foreseeable or Eyes On Eyecare had been advised of the possibility of Attendee incurring the same.
- Indemnity. Attendee shall indemnify and defend Eyes On Eyecare for any costs, losses, or damages that Eyes On Eyecare pays to the venue or any other supplier connected to the Event following (i) any breach by Attendee of the relevant rules and regulations relating to the venue or Event; (ii) any damage done to that venue or accommodation or any equipment thereon by Attendee, including, but not limited to, any equipment provided for the Event; and (iii) any injury suffered by any person, animal or item caused by or in consequence of any act or omission of Attendee.
- Rights of Use. By agreeing to the Terms and Conditions, I hereby give Eyes On Eyecare permission to use any and all of my voice, image, and likeness, with or without using my name, in connection with the products and/or services of the Company, for the purposes of advertising and promoting such products and/or services and/or Eyes On Eyecare, and/or for other purposes deemed appropriate by the Company in its reasonable discretion, except to the extent expressly prohibited by law. All digital image(s) and/or video files are wholly owned by Eyes On Eyecare and use of these assets has been granted without compensation (fees, royalties, special credit).”
- Governing Law. These Terms and Conditions are governed by California law and the courts located in San Diego, California shall have exclusive jurisdiction to determine any disputes which may arise under or to them.
- Consent. By agreeing to the Terms and Conditions, I certify compliance to these terms on behalf of the registered attendees and/or their organizations.
By agreeing to the Terms and Conditions, I further represent that I have the full right, power, and legal capacity to enter this Agreement on behalf of Attendee(s).
