Diagnostic errors can cause serious harm or death. A 2023 Johns Hopkins study concluded that an estimated 795,000 Americans become permanently disabled or die annually because of misdiagnoses. Vascular events, infections, and cancers topped the list for serious harm. The top 5 diseases were stroke, sepsis, pneumonia, venous thromboembolism, and lung cancer. A new Massachusetts Department of Public Health report of 678,382 deliveries noted that dangerous complications from Labor and Delivery nearly doubled from 2011-2020, rising an average of 8.9% each year. Diagnostic failures are mostly due to faulty clinical reasoning, poor decision-making, an uncaring attitude, physician burnout, and inexcusable unawareness of new advances in medicine. The latter is especially relevant to the practice of obstetrics given advances in medical genetics that impact preconception, perinatal, and postnatal care, and which include the use and limitations of cell-free DNA screening, microarrays, whole exome sequencing, and whole genome sequencing. Cognitive biases, of which there are over 100, directly influence clinical decision-making and receive insufficient attention in the training of physicians. Mindless medicine, a risk-taking personality combined with a non-caring attitude further compound and invite catastrophic consequences. All of these considerations bear on the standards of care and avoidance of medical malpractice, an important focus of this conference.
