ABIM MOC:
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 7.00 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program.

Educational Needs:
The program is intended to educate rheumatologists on questions concerning the pathogenesis of both Rheumatoid Arthritis (RA) and Periodontal Disease (PD). The sessions will cover why oral health is an important consideration for RA patients, how “red complex” pathogens participate in driving RA pathogenesis, if PD can make RA symptoms worse/better, the role ACPAs play in driving the pro-inflammatory component of both RA and PD and how the Shared Epitope fits into this process. The program will also cover citrullination and what other kinds of post-translational modifications may be involved, and how neo-antigen formation is involved in the auto-immune component of PD and RA disease pathogenesis.

Educational Objectives:
At the conclusion of the TRS 2019 Annual Scientific & Business Meeting, attendees will be able to:

• Identify what therapy should be used next after failing a first TNF inhibitor.
• Assess approaches to managing anti-TNF cycling vs. switching to a different mechanism of action agent.
• Explain the difference between primary and secondary non-responders.
• Describe which therapies are more effective in seropositive RA patients.
• Describe the cytokine networks and cell populations that promote inflammation in the psoriatic plaque and musculoskeletal sites in psoriatic arthritis.
• Outline key cytokines responsible for both pathologic bone resorption and new bone formation in psoriatic arthritis.
• Outline the rationale and potential candidates for combined biologic therapy to improve outcomes in PsA.
• Identify the clinical features of ANCA-associated vasculitis.
• Describe the approach to diagnosis for ANCA-associated vasculitis.
• List the treatment options for ANCA-associated vasculitis.
• Outline the key domains in psoriatic disease.
• Review evidence regarding the efficacy for specific biologics in certain domains such as skin, axial disease and enthesitis.
• Discuss strategies to treat psoriasis patients at high risk for arthritis prior to the onset of musculoskeletal symptoms.
• Describe the diseases that can be associated with large vessel vasculitis.
• Identify the clinical features associated with giant cell arteritis and Takayasu arteritis.
• Discuss the treatment approaches to giant cell arteritis and Takayasu arteritis.
• Explain the importance of addressing oral health issues along with standard care.
• Discuss the potential for a role for ACPAs in driving disease pathogenesis in addition to their use as an RA diagnostic indicator.
• Explain the role for the shared epitope (SE) in the generation of the ACPAs.
• Identify the different enzymes that are responsible for generating neo-epitopes through the post-translational modification of host proteins, whether endogenously produced or generated by pathogens.
• Identify the only prokaryote that produces a peptidyl arginine deiminase, and explain how this enzyme functions.
• Identify other prokaryotes that have been suggested to be involved with RA/PD pathogenesis and identify the toxin suggested to drive this process. Explain the role that neutrophils may play in this pathogenesis.
• Discuss the nature of the ACPA (CCP2) diagnostic assay.
