These presentations will highlight how health care organizations are taking innovative approaches to care delivery and payment, and managing complex care for patients with chronic disease.
Clinical-Community Connections:
To maximize their impact on health outcomes and lead the movement toward accountable, population-based care, health care organizations need to align their population health management and community health strategies. These presentations will demonstrate systemic approaches linking clinical and community-based strategies to address the medical and social determinants of health for their patients and community.
Collaborating for Community Innovation:
Health care organizations are reaching outside their walls and building on their role as purchasers and employers to develop innovative and collaborative strategies to improve healthy equity and well-being in the communities they serve. These presentations will share innovative practices of how health care organizations collaborate with community stakeholders to drive development and help individuals and communities reach their highest potential for health and well-being.
