Program Description:
This dynamic and interactive program will focus on enhancing your knowledge, skills, and competencies to effectively communicate and collaborate with the primary care provider team and patients with diabetes. Evidence shows that effective patient-provider dialogue enhances clinical efficiencies, facilitates patient empowerment and engagement, and contributes to better patient adherence. Topics planned for discussion are ambulatory glucose profile (AGP) with interpretation, AGP for diagnostics and therapeutics, UVA’s approach to merging clinical and engineering perspectives in diabetes management, using continuous glucose monitoring (CGM) to enhance lifestyle changes, incentivizing technology use in practice, and integrating CGM data in electronic medical records. There will also be a moderated discussion, a hands-on activity using glucose sensors, insulin pumps and pens a round table discussion and solutions of case reviews.
Learning Objectives:
At the completion of this program, participants will be able to:
• State the background, rationale, construction and nomenclature of ambulatory glucose profile and how to measure its various components
• Describe the process for sequencing, interpreting and completing a longitudinal analysis of AGP reports and how it is linked with patient behavior modification and continuous glucose monitoring
• Recall the role technology plays in glucose monitoring, glucose sensing and insulin delivery, what can be learned about diabetes management from mathematical and simulated models and how clinical decisions and patient performance are impacted by real-time continuous glucose monitoring
• Explain the principals for lifestyle change as it pertains to continuous glucose monitoring and the impact of helping type 1 and 2 diabetic patients improve their glycemic control
• Demonstrate aspects associated with continuous glucose monitoring and the insulin infusion pump/pen
Interpret continuous glucose monitoring data and document it for reimbursement
• Discuss how continuous glucose monitoring data are, or can be, integrated into the electronic medical record.
