2018 ACP Michigan Conference Advancing Advance Care Planning is organized by Michigan Primary Care Consortium (MPCC) and will be held from Oct 11 - 12, 2018 at Crowne Plaza Lansing West, Lansing, Michigan, United States of America.
Our Purpose:
For attendees, from new to maturing ACP programs, this conference will challenge us all with topics from MI-POST, ACP laws, Reimbursement, Dementia, Community Engagement, Shared Decision-Making, Conversation Skills, Dementia, Setting-specific tracks and much more!
Course Objectives:
• To equip attendees to implement ACP in their setting
• To promote shared decision-making in all stages of ACP
• To enhance the ability to honor individuals’ wishes
Pre-Conference PACE Workshop is will be held on Wednesday, October 10, 2018 at 1:00 p.m. - 4:00 p.m.
