Medicines Optimisation

This conference focuses on implementing the key recommendations from the new NICE Guideline including putting the principles of medicines optimisation into practice, involving people in decision making about medicines, sharing information to avoid errors (including implementing the Patient safety alert on risks arising from breakdown and failure to act on communication during handover at the time of discharge from secondary care), medicines reconciliation, medication review and measuring and monitoring medicines adherence.
