The prevalence of diagnostic error, defined as wrong, missed or delayed medical diagnosis, occurs in 10-15% of cases, leading to an estimated 40,000 – 80,000 deaths annually in the Unites States. Diagnostic error is responsible for billions of healthcare dollars for inappropriate care, and is the leading cause of medical malpractice claims.
