This activity trains physicians, providers and other care team members registered for the AHA/AMA joint Target: BP™ initiative, on the “Partner with patients, families, and communities” component of the AMA MAP Quality Improvement Framework. MAP stands for Measure Accurately, Act Rapidly, and Partner with Patients.
Learning Objectives:
At the completion of this course, the learner will be able to:
• Describe evidence-based use of self-measured blood pressure (SMBP)and best practices for engaging patients in self-management of their blood pressure.
• Recognize importance and how to employ evidence-based collaborative communication strategies with patients with hypertension.
• Access available supporting tools and resources to support use of SMBP and collaborative communication strategies.
