Leading clinical faculty from Harvard Medical School, Massachusetts General Hospital, (MGH) and Spaulding Rehabilitation Network have come together to develop this new CME program.

It offers 360-degree education to manage patients with multiple comorbidities. Specifically, it:

• Delivers updates and state-of-the-art practices for examination, diagnosis, and management of the 20+ conditions most commonly seen in medically complex patients, including heart failure, diabetes, sepsis, cancer, neurologic and immune-modulated diseases.
• Advances your understanding and ability to manage these conditions, not just as siloed diseases, but as interactive physiologic systems and in so doing, secure the best possible outcomes for your patients. We include comprehensive strategies for:
    Risk mitigation
   ○ Avoiding unintended drug-drug interactions
   ○ Navigating competing treatment approaches and complications
    Reducing unplanned hospital readmissions
• Provides resources and best practices for cross-specialty collaboration and care coordination for medically complex patients.

Learning Objectives:
Upon completion of this course, participants will be able to:
• Identify emerging topics in the care of medically complex patients, including sepsis, renal failure, respiratory failure, cardiac arrest and failure, delirium, and ICU-rehabilitation
• Implement evidence-based guidelines and best practices for common clinical conditions in the medically complex patient using an interactive, case-based format
• Safely manage patients with complex medical and post-surgical care needs across the spectrum of care in a cost-effective manner
• Employ studied and proven medical and technological options for diagnosis and treatment of illness during hospital stay
• State methodology to prevent common complications of chronic hospitalization, including malnutrition and metabolic derangement, pressure ulcers, physical deconditioning, thromboembolic disease, bladder and bowel complications
• Develop strategies for safe and efficient care transition for oncology, transplant heart failure, and other medically complex patients to reduce hospital readmission rates
• Refine communication strategies to discuss medical and functional outcomes with patients and families, and to clarify goals of care
• Identify common ethical dilemmas in the care of the medically complex patient, including chronic illness and debility, and associated mental health conditions
• Explain strategies to manage risk in medically complex patients, and develop quality metrics to benchmark individual patient and hospital/system-wide outcomes
• Utilize risk control and performance improvement methodologies across the continuum of care
Interpret evolving evidence-based protocols and best practices for prehabilitation interventions prior to elective major surgery
• Expand their use of innovative diagnostic technology, such as point-of-care ultrasound and bedside procedures in medically complex patients
