Critical Care Management - 3 ways to manage your patients health

Critical Care Management - 3 ways to manage your patients health

Posted by Monika Mudumba and Priya Korrapati on January 31, 2020

Critical care helps people with life-threatening injuries and illnesses. According to research by CMS (Centers for Medicare and Medical Services), 117 million Americans are diagnosed with at least one chronic issue. The study also states that by 2025, half of the American population will suffer from a chronic disease. It is of prime importance for a physician to discover ways and assist patients in managing their chronic conditions.

Here are the three beneficial critical care management practices for physicians.

Include CCM as a fundamental part of clinical practice

Involve expert supervisors in practice. Guide them to use the same type of equipment as the internal administrative team uses. Share the documented patient information among the physicians. Build a close relationship with the CCM provider to set an active program service to meet the goals and individual needs of a patient.

Involve a key clinical team member

Request an expert physician or a senior nurse to lead the program. The responsibilities include patient enrollment to the data collection. You can do this by applying EHR integration to ensure,

  • Establishes a proper clinical workflow
  • Reduces service delay of the patients caused due to poor internet
  • Vanishes the trouble syncing the records to obtain the test results
  • Help patients to keep up with their treatment plans

Strategize patient communication

Befriend your patients to share their odds and ends of life. Help them ease the pain.

You can use the "EDUCATE" model to convince your patient for therapy,

Encourage

Compliment and acknowledge your patients for seeking treatment. Setting up for a therapy appointment is a vital move towards ensuring holistic health. Considering the ethical and confidential boundaries, it is also essential for you to highlight the positivities of a patient.

Support and respect the patient's efforts to longing therapy.

Discuss

Develop an encouraging tone to draw most of the information from a patient. Direct a conversation that adds the details on how they've maintained a healthy lifestyle. Also, develop an [aspect?] of empathy; it would be easier for a patient to open up about their lifestyle and habits. Likewise, it would also be easy for you to draw information on alcohol use, recreational drug use, and other factors.

Consult

Get through the bits and pieces of patient information. Check on the patients' medical history through EMR (Electronic Medical Record).

Assert

Direct your patients to get asserted and firm of their health. Ensure that the statements provided by a patient must be clear and concise. Make your patient address a medical problem clearly, and furnish a clarification.

Teach

Access the risk factors, protective factors and put forth the questions to draw a full picture of a patients' health condition. Address an understandable roadmap for a patient that paves a path towards better health.

Empower

Assist and communicate with your patients —elaborate a plan of action to sustain a healthy lifestyle. Explore and present the strategies to maintain a balance of their health.

Encourage and Educate your patients. Have some time and space to work through a patients' helplessness. It will not only build patient engagement but also reduces physician burnout.

Critical care helps people with serious injuries as well as illnesses. This can be due to problems such as complications from surgical treatment, accidents, infections, and serious breathing problems. This requires close and constant attention from a team of specially trained medical professionals. Intensive care is usually performed in the intensive care unit or at the trauma center. This is exactly the topic that I talked about in college when I was writing a dissertation, you can see it at my dissertation writing service, just click here . I even started helping students and working as a remote writer. Therefore, I will be happy to help you. I will also introduce you to my dissertation. I wrote it for more than a year, this topic was very interesting for me. I can advise you on this issue.
